Job Description

Job Summary:

The Hospice Nurse Practitioner will provide will support VNA Hospice by providing face to face visits to patients that need to recertify for hospice care. The Hospice Nurse Practitioner will ensure compliance with established professional and accreditation standards, practices and regulatory requirements.

Duties and Responsibilities:

  • Conducts a comprehensive review of the hospice plan of care and interval progression of illness
  • Conducts a comprehensive assessment of symptom burden and effectiveness of current treatments to provide comfort
  • Formulates a hospice diagnosis based on disease trajectory and the hospice LCD Criteria
  • Assesses eligibility for Hospice Care

Interventions

  • Recommends screening tests that are based on goals of care and risk/benefit/burden ratio and if needed for determining ongoing hospice eligibility
  • Makes appropriate recommendations to the hospice team based on the symptom burden assessment
  • Recommends plan for recertification on hospice

Communication

  • Provides effective discussions of recommendations to the hospice clinical manager or the hospice medical director
  • Completes a comprehensive documentation in the electronic record for the face to face visit

Administrative

  • Electronic record documentation for face to face encounters

Able to Describe the Following:

  • Infection control
  • Bag Technique
  • Standard Precautions
  • Developmental issues through the life cycle

Requirements:

Advance Practice Nurse from an accredited university.

At least one (1) years previous experience providing Hospice and/or Palliative Care. Must possess the ability to coordinate a high level of activity under a variety of conditions and constraints. Advance Certification in Hospice and Palliative Care preferred.

RI NPP License, valid Driver’s license and auto insurance, BLS CPR
