Are you licensed pharmacist looking for a new opportunity? Our Home Care team is looking for a patient-centered and passionate pharmacist to join their team! We are an exciting area to practice pharmacy at the top of your licensure. Our staff are involved in the clinical care of patients and coordinate with multidisciplinary teams at hospitals or physician practices to provide recommendations in order to obtain the best outcomes possible for our patients.

We have just moved into a brand new facility to design the most state of the art compounding facility in the system!

• Monday - Friday
• 8:30am to 5pm
• Saturday in office every 4 - 5 weeks 
  On-call approximately 4 nights/month

Overview

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.

The Home Infusion Therapy Department of BJC Home Care Services works closely with physicians, patients and caregivers to enable patients to remain or return home while receiving intravenous medications. Our services include 24 hour on-call pharmacy and nursing services, comprehensive patient education materials, patient assessment and drug administration monitoring, and coordination of professional services including home care nursing and laboratory services.

Preferred Qualifications

Role Purpose

Assess, monitor, initiate and modify medication use according to patient condition and age specific criteria. Ensure prompt and accurate medication preparation and dispensing. Prevent, direct and resolve medication-related issues. Supervise and direct supportive personnel. Counsel and educate to promote optimal drug therapy. Direct and assist in the procurement, storage, distribution and disposal of medications.

Responsibilities

• Directs and manages workflow including oversight of technicians, interns and volunteers ensuring the accuracy of their work in accordance with departmental procedures and Federal and State regulations.

• Participates in departmental meetings and on committees at the health service organization, as appropriate.

• Serves as medication therapy resource for activities such as drug information, training of staff, transitions of care and Medication Therapy Services.

• Evaluates medication orders for appropriateness with regard to patient condition, organ function and concomitant therapies to promote optimal patient outcomes. Documents all activities and communication as appropriate.

• Recognize opportunities and offer solutions to maximize efficiency and promote safety within the medication use process.
